State Care Coordinator

The primary responsibilities of this position are:

  • Care coordinator will be assigned to a treatment facility during designated times to engage clients.
  • Conduct intake screening which includes information about health needs, housing, employment and other requested services.
  • Conduct assessment for clients’ eligibility for enrollment into the Access to Recovery program.
  • Enroll uninsured clients into Primary Adult Care (PAC) and other public health insurance programs and assist them in choosing an MCO and identifying a primary medical provider.
  • Refer to relevant programs that assist residents of Baltimore City, Baltimore County or Anne Arundel County as required.
  • Conduct and document a minimum of two case management units per month per client.
  • Active and persistent outreach
  • Document on internal and state database all contacts, with clients and/or providers.
  • Enter data and charges into agency and state data base on a daily basis as appropriate.
  • Provide intensive case management to clients that are enrolled in substance abuse treatment.  This includes making referrals for housing, job training and other services.
  • Encourage clients to engage in outpatient substance abuse treatment after inpatient or residential treatment has been completed.
  • Attend follow-up meetings held at treatment facilities, patient homes/transitional houses/recovery houses, at the HCAM offices or other facilities as agreed upon by the Care Coordinator and client.
  • Meet with clients on an as-needed basis to provide ongoing assistance accessing services and coordinating care.
  • The Care Coordinator will maintain a relationship with the client for six months to guarantee successful completion of the program.
  • Attend administrative and management meetings at HCAM as required.
  • Perform other duties and assignments as required.

Minimum Qualifications

  • Associate Degree or equivalent. Bachelor’s degree preferred
  • Certified Application Counselors (CAC) certification requirements must be completed within the probationary 6 month period
  • Previous experience with case management
  • Previous experience with fee for service programs
  • Previous experience with SMART data base
  • Previous experience with substance abuse population/programs
  • Excellent organizational skills
  • Proven ability to meet deadlines
  • Ability to effectively engage clients and connect them to resources
  • Ability to comprehend and manage documentation in multiple databases
  • Ability to manage of caseload of 100+ clients
  • Ability to problem solve and demonstrate initiative in approaching complex cases
  • Required computer literate specifically MS Word, Excel, Access, and Outlook.
  • Excellent oral and written communication skills required.
  • Demonstrate positive attitude in work duties and interactions with peers, clients and funders.
Physical Requirements: The ideal candidate must be able to: Ambulate 3 blocks or more
