Addiction Outreach Services
For decades, Baltimore has had one of the highest rates of heroin addiction in the nation and the demand for heroin treatment exceeds supply. HCAM has several programs that not only address common barriers that prevent city residents from accessing treatment for their addiction we also collaborate with treatment programs, health centers, and doctors to make sure those in treatment are getting any additional healthcare services they need.
Care Coordination
The Care Coordination Program provides short term case management and ombudsman services to recipients of Medicaid, Medical Assistance for Families, and the Maryland Children’s Health Program.
Eligibility
For Baltimore’s uninsured it starts with a phone call to our Eligibility Hotline at 410-649-0500 and more often than not it ends with health care coverage! Our dedicated Eligibility staff process applications for several of Maryland’s public health care programs including Medical Assistance for Families, Maryland Children’s Health Program, and the Primary Adult Care Program. Homeless Services
Our Homeless Services program is dedicated to reducing the number of individuals experiencing homelessness in the city.
Immigrant Services
Our Immigrant Advocates assist Baltimore City’s immigrant population by providing interpretation services,
MCHP, Medicaid, and Emergency Medical Assistance information, and local resource referrals. Formerly the Hispanic Team, we have expanded services beyond the Hispanic community to include all Baltimore immigrants.
MATCH – Making All The Children Healthy in foster care.
MATCH provides medical case management and health care coordination for all children entering the foster or kinship care system. Program staff ensure enrollment in Medical Assistance and identify with the foster or kinship care parent an appropriate managed care organization.
Maternal and Child Health Programs
Maternal and Child Health programs serve Baltimore City pregnant women, their babies, and families by helping them navigate the health care system to get the health care services they need. Operation Care
The Baltimore city fire and ambulance service never turns down a call and must respond, even when many of the calls come from a handful of residents who called over and over again.
Many of those calls tie up medics at the scene and at hospitals. Emergency management crews refer to those patients as frequent fliers, and they’ve identified 115 of them, including Margaret Joyce.
BHCAs Operation Care is making a difference and saving the city money by connecting “frequent fliers” to the care they need in the community.

Medicaid and a variety of other health insurance programs and services may also be available.
