Through the Empowerment Zone Project, HCAM is outreaching clients either enrolled in or seeking drug treatment who live within the Empowerment Zones. These zones include the neighborhoods of Poppleton, Sandtown, Pigtown, Washington Village, and Historic East Baltimore. Advocates work closely with clients to make sure they are accessing all benefits that might enable them to remain in recovery, including public entitlements, housing, job training and education. Clients are provided case management services to ease the transition from one stage of treatment to another – whether it be contemplating treatment for the first time or moving from active treatment to supportive services.

Treatment Advocates can assist with the following:

  • Applying and enrolling in the Primary Adult Care (PAC) program
  • Obtaining necessary documentation required for enrollment into PAC and other public programs
  • Helping clients navigate the Medicaid system, including enrolling in a health plan and locating an appropriate treatment provider
  • Referring clients to their local Department of Social Services for other available public benefits
  • Scheduling Primary Care Provider office visits and conducting follow-up if necessary
  • Assisting patients with barriers to care such as transportation and co-pays
