The HOPE Project is a HCAM outreach initiative providing Primary Adult Care (PAC) Program eligibility assistance and linkage to community resources and suports to those in Baltimore City who are receiving addiction treatment. Following our successful community-based model, the HOPE Project outsources treatment advocates to 10 methadone, drug free, and adolescent treatment sites where they help to expand access to care and provide supports for clients who are already enrolled in addiction treatment.
HOPE Project Treatment Advocates can assist with the following:
- Applying and enrolling in the Primary Adult Care (PAC) program
- Obtaining necessary documentation required for enrollment into PAC and other public programs
- Helping clients navigate the Medicaid system, including enrolling in a health planĀ an appropriate treatment provider
- Referring clients to their local Department of Social Services for other available public benefits
- Scheduling Primary Care Provider office visits and conducting follow-up if necessary
- Assisting patients with barriers to care such as transportation and co-pays
