Care Coordination for Women & Children
Our Care Coordination for Women & Children project approaches addiction as a chronic disease that affects the entire family.
Through this program, HCAM offers care coordination services to women who have at least one child in their custody and who are enrolled into the Women and Children Supportive Housing Project by Behavioral Health System Baltimore (BHSB). HCAM Care Coordinators work closely with these clients and their children to help them get the whole-person services they need to complete outpatient substance abuse treatment. Linkages include referrals to insurance, health care, employment, education, housing, transportation, legal services, and more. Our Care Coordinators follow up with clients at least four times a month to adjust the care plan and track progress.
