
HealthCare Access Maryland’s Care Coordination program provides short term case management to hard to reach HealthChoice patients who have missed medical appointments or are having problems following medical recommendations.
Referrals generally come from HealthChoice Medicaid providers and health plans.
Services are primarily provided face-to-face in the home by registered nurses, licensed social workers, and care coordination associates. The goal is to establish or restore the health plan/Primary Care Provider link to the enrollee by removing any barriers to care. Services may also be provided to other household members.
Services may include some or all of the following:
- Education on accessing care in HealthChoice, the importance of preventive health care, the use of health plan Member services, the HealthChoice Enrollee Action Line, and other needs based on assessment
- Linkage to the health plan, Primary Care Provider, Family Planning, and other appropriate community resources such as WIC, daycare, transportation, social services, and school health
How Do I Make a Referral?
Referrals are received via the Local Health Services Request form (DHMH 4582). Referrals are initiated by the health plan. All requests should be sent to:
