In partnership with the Division of Pretrial Detention and Services, HCAM has two case managers placed at the Baltimore City Detention Center to assist inmates with accessing public benefits upon release. This program includes pre-release services to inmates including application assistance for insurance and the Supplemental Nutrition Assistance Program (SNAP), and linkage to care. HCAM Liaisons screen identified inmates for eligibility and begin benefit applications for prioritized benefits, provide crucial transition services, and follow-up on all scheduled appointments for somatic and behavioral health care while continuing to provide referrals for supportive services.
