Population Health

HCAM’s Population Health Program has been established to focus on the health outcomes of the Baltimore City, Baltimore County and Anne Arundel county residents, specifically addressing the determinants of health (i.e., medical care systems, social environment and physical environment) and the biological impact these determinants have on the sub-populations within these areas. With a dual focus of increasing access to care and assistance with health system navigation for the socioeconomically disadvantaged, the Population Health Program provides health promotion and care coordination across interdisciplinary settings to promote the overall health of this population through health advocacy and inter-agency collaboration.
