Care Coordination

The Care Coordination Program (CCP) is an outreach program that provides short-term care coordination services to Baltimore City residents. The Care Coordination Program assists Medicaid/HealthChoice eligible beneficiaries in accessing and using their Medicaid benefits. Most importantly, CCP links clients to the Maryland’s Managed Care Program, HealthChoice. We educate clients about health benefits, linkage to primary care and specialty services, work to improve birth outcomes in Baltimore City with innovative prenatal outreach strategies and strive to protect our city’s most vulnerable populations by providing access to health services and community resources.

Annually, the Care Coordination provides outreach and care coordination services to over 12,000 Baltimore City residents.
